The Anchorage Pioneer Home is looking for a highly motivated, goal oriented individual that is interested in working as a team player to fill ou! r Social Services Associate II position.
This position coordinates admissions to the Home and assists families and clients with the transition to assisted living. Provides social work services to current residents of the Anchorage Pioneer Home as well as prospective residents.
Key Responsibilities:
Coordinates admission to the Anchorage Pioneer Home. This includes but is not limited to maintaining the Home's wait list, scheduling and providing tours of the Home, working with nurse managers to provide initial functional assessments of potential residents, providing orientation to all new residents of the Home, and communicating with all Home departments on time lines for new arrivals.
Collects admission information and assists new residents and families with initial paperwork requirements necessity by the move in process, which include physician's statements, physician medication orders, residential service contracts, in house assessment forms, an! d other forms required by State licensing.
Under the guida! nce of the Home's Protective Services Specialist III, assists with resident social work related issues. These can include, but are not limited to, guardianship/conservatorship power of attorney concerns, permanent fund dividend applications, and family communications.
Works closely with medical records personnel to ensure all initial required paperwork and documentation is completed.
Has understanding of all medical records needs in the Home which include but are not limited to Division reporting requirements and retention schedules.
Minimum Qualifications:
High School diploma or the equivalent; and one year of paraprofessional experience (any level) providing assistance, information or referrals to the public related to social services, health care or legal services.
Substitution: Post-secondary education from an accredited institution in the social, behavior or health sciences, law or a closely related field may substitute for t! he required experience on the basis that three semester/four quarter hours equals one month of experience.

Some positions require a valid Alaska driver’s license, a background check and fingerprinting. These requirements will be indicated at the time of recruitment.
“Equivalent†includes a G.E.D., or completion of any basic adult education course of 480 class hours (16 weeks at 30 hours per week), or six months of work experience performing duties related to the job class, or twelve months of work experience involving some routine tasks related to the job in question, or highest grade of school completed plus amount of work experience which totals 12 years.
