SUMMARY OF JOB DUTIES FOR NURSE II

Provide the professio! nal nursing expertise necessary to make determinations regarding client eligibility for Children with Complex Medical Conditions (CCMC), Older Alaskans (OA), and Adults with Physical and Disabilities (APD) Medicaid waiver services, nursing home stays, and Personal Care Services (PCA).
Perform client assessments based upon reports from a doctor along with records and medical provider to fulfill Medicaid requirements by approving or denying services eligibility.

Approve or deny the type and level of nursing care identified as necessary in the client Plan of Care (POC) submitted by providers.

Conduct quality assurance activities.

Provide consultative assistance to Adult Protective Services and professional level staff consisting of both permanent positions and non-permanent position, which may include attending home visits to assess and document the client’s physical/emotional status.

Make recommendations related to the care needs rega! rding the client’s health and safety welfare. FUNCTIONAL DUT! Y STATEMENT FOR NURSE II

This position is responsible for performing a face-to-face professional nursing assessment of applicants requiring knowledge of nursing principles and procedures clients to determine whether or not applicants and participants meet the requirements for OA/APD/CCMC waivers and/or PCA services, that the nursing services on the plan of care are necessary, of the proper type and level for the client to receive adequate care to ensure client health and welfare and meet Medicaid eligibility requirements, and also approves Nursing Home authorizations and authorizes Medicaid payment for nursing home services.

Conduct home and/or institutional care facilities visits to observe and evaluate the client’s physical and emotional health condition, behavior. Clients/applicants are also evaluated for the Personal Care Attendant Program with a personal care services plan developed authorizing identified care/service. OA/APD/CCMC Medicaid Waivers e! nable an adult or child whose needs would otherwise result in placement in a nursing home or intermediate care facility to remain safely at home or in a community setting. Waiver services are provided to individuals whose level of functioning qualifies them for nursing home or intermediate care level of care. If authorized, services may include in-home services, specialized medical equipment, environmental modification, assisted living, habilitation, transportation, and respite among others.

Initiate and complete nursing assessments on clients and conduct nursing expedite assessments consistent with established criteria. May need to respond to medical emergencies and initiate EMS response as needed. Determine client eligibility for the OA/APD/CCMC Medicaid Waiver Program by using nursing expertise to review, approve or deny the full range of routine to complex client assessments. Assessments include a wide variety and type of health conditions such as stroke, quadr! iplegia, respiratory disease, cardiac disease, etc. Coordinate process ! and track screenings from various referral sources, evaluate assessments, determine eligibility for nursing facility level of care, and make referrals to appropriate programs utilizing the most appropriate funding source. Clients may have technology dependence and use advanced medical equipment such as ventilators, oxygen and oxygen related equipment and feeding tubes; or may use a variety of prosthetic and/or assistive devices such as walkers, wheelchairs, canes, to name a few. These factors are evaluated for consideration of the specific program eligibility requirements and of the client’s assessed needs. Collaborate with medical health care providers, clients, families, care givers, and legal representatives to obtain an accurate clinical picture of client needs and how those needs relate to the State regulations for requested services. Coordinate with other Divisions and units to maximize resources and minimize duplication of funding. May need to respond to medical em! ergencies and initiate EMS (Emergency Medical Systems) response when client health and safety is at imminent risk.

Conduct nursing expedited assessments consistent with established criteria in situations where the client’s condition has deteriorated and there is no one to provide care; clients with a terminal diagnosis and no support, etc. Onsite quality assurance evaluations and reviews during reassessments. Documents the observations and assessment to establish the nursing services and personal care services for the plan of care and service plan that is essential in order to confirm that the proper type and adequate level of care is chosen, and it meets the Medicaid eligibility requirements to ensure client’s health, welfare and safety.

Attends Fair Hearings and Pre-hearings regarding level of care issues; maintain communication with Fair Hearing Authority, state Attorney General´s office, and Allied Computer Services (ACS); maintain documentation f! or Fair Hearing Process and decisions. Provide professional research an! d analysis to develop information to support decisions to deny Medicaid waiver services or nursing home services to a recipient and participate in the recipient´s appeal process. Provide information to the provider agencies, recipients, and family members regarding Medicaid state and federal regulations, policies and guidelines.

Minimum Qualifications:
Graduation from a school of nursing accredited by the National League of Nursing or license to practice as a Registered Nurse in the State of Alaska.
AND
One year of entry level professional nursing. This experience is equivalent to Nurse I with the State of Alaska.
Other combinations of training and experience will be evaluated on an individual basis for comparability; however, there is no substitution for the required training.
Special Requirements: Eligibility for licensure to practice as a Registered! Nurse in the State of Alaska is required.
Some positions are located in institutions providing care for psychiatric or developmentally disabled individuals and require one year experience in these specialized fields.
Note: For positions in this job class a background investigation including fingerprinting is required.
