The Anchorage Pioneers' Home is looking for a highly motivated, goal oriented individual that is interested in working as a team player to fill our Protective Ser! vices Specialist III position.
This facility is an Eden Alternative facility. The selected candidate may have contact with birds, cats, and dogs.
Important Duties of this position:
Participates with Nursing staff in the development, implementation and evaluation of behavioral care plans.
Provides individual counseling, group counseling, and general psychological consultation and assessment services with an emphasis on Alzheimer´s Disease and related disorders.
Provides monthly supervision, oversight, and approval on work product in the Social Services Department at the Alaska Veterans and Pioneer Home in Palmer, AK.
Assists residents and families to apply and re-certify for financial assistance. Responds to requests from Admissions Coordinator to assist families and residents with admission transition.
Orients new residents and families to the Home Policies and educates them of their rights. Instructs and collects information re! garding advance directives. Makes frequent contact with new re! sidents during adjustment period. Assists resident, family and staff if resident is having difficulty adjusting to the Home environment.
Assists residents and families with Durable Power of Attorney or guardianship and conservatorship forms as necessary. Explains ramifications of all such actions or lack of action.
Completes and files petition forms for residents who are not competent to manage their own affairs and have no family or other support. Represents the Home at court guardianship and conservatorship hearings when the Home is the petitioner. Educates and provides technical assistance to staff and residents regarding the role and responsibilities of the Attorney in Fact, guardian and conservator.
Advocates for residents in legal matters and care planning. Acts as liaison between residents, families, guardians, conservators and other individuals with the legal right to make decisions regarding a resident´s care.
Participates in the ongoing staff trai! ning and education program including orientation of new staff, yearly updates for staff on resident rights, other topics related to team development and needs of residents and families.
Provides conflict resolution on issues involving family members and the Home.
Conducts an investigation to identify possible resident abuse or violation of resident rights when a report of harm is received. Submits written investigation report to the Administrator.
Counsels residents and families through grief and loss related to terminal illness, aging, and death and dying issues. Facilitates group therapy and support groups for residents and resident family members.
Represents the APH at community meetings and meetings with social service agencies. Networks with other social service agencies to educate service community of APH policies, procedures and practices.
Day to day assistance to residents with personal matters, i.e., applying for resources such as legal assistan! ce, medical assistance and community services. NOTE : A pplicants who m! eet the Minimum Qualifications and have a Master's of Social Work or a Social Worker License will be given preference.
Minimum Qualifications:
A bachelor’s degree from an accredited institution with a major in a social, behavioral or health science, law or a closely related field;
AND
One year of full-proficiency level professional experience in child, adult, family, or group social work or licensing experience. This is met by experience as a Protective Services Specialist II or Community Care Licensing Specialist I with the State of Alaska, or the equivalent elsewhere.
Substitution:
Four years of any combination of postsecondary education (3 semester/ 4 quarter hours equal 1 month of experience) in a social, behavioral or health science, law or a related field and/or paraprofessional experience (any level) providing assistance, information or referrals to the public related to social services, health care or legal services ! may substitute for the bachelor’s degree.
Special Note:
Some positions require a valid Alaska driver’s license and/or a background investigation including fingerprinting. These requirements will be indicated at the time of recruitment.
Some positions require specific licensure due to funding restrictions.
